| The teacher, Maura Larkins, discovered that money was being taken out of her paycheck without her approval. She asked Lowell Billings to provide her with a copy of the document that triggered the withdrawals. He refused to provide it. After repeated requests, he finally produced the document at right. |

| Anthony Pavia told the teacher to cross out the name of the company she didn't want, and initial her cross out. She did. Next, he crossed out the company she wanted, and wrote it on the first line. He asked her to initial that change. She did. Then, after he had left his meeting with the teacher, he wrote in the name of the insurance company she didn't want on the second line. |
| The scanned document below is exhibit A in the evidence that a man who was authorized by CVESD district and the teachers union to come into classrooms and give financial advice. The teacher never asked for an account with Life Insurance of the South West, but Anthony Pavia had already written it in on the top line of this document before he showed the document to the teacher. He had also written in the name of the company the teacher wanted to do business with on the second line. When the teacher asked for another form, Pavia claimed this was the only form he had in his possession. |
